The WHO's 2020 physical activity guidelines contain two recommendations for adults, not one. The aerobic one — 150 to 300 minutes of moderate activity a week — gets all the attention. The second is muscle-strengthening at moderate or greater intensity, involving all major muscle groups, on two or more days a week.

It is a separate line item. Doing the walking does not discharge it, and it is the half that desk workers reliably skip, usually because strength training is coded as an aesthetic pursuit rather than a health one.

Why this population specifically

Sitting is not neutral for the musculoskeletal system in the way that lying down roughly is. It holds some tissues in a shortened position under low load, holds others lengthened and unloaded, and asks almost nothing of the large muscles of the legs and hips for hours at a time. Be careful with how far that goes, though. The popular version — that sitting causes specific muscles to switch off, which causes specific pain, which specific corrective exercises fix — is a much stronger causal chain than the evidence supports. The musculoskeletal literature on computer work is inconsistent, and targeted corrective exercise is one of the weaker areas within it.

What survives is the modest version: a body that spends nine hours in one configuration benefits from being loaded in others, and general strength across all major groups is what the guideline recommends and what the evidence best supports.

"All major muscle groups" is doing real work in that sentence. It is a general conditioning recommendation, not a prescription for the bits that hurt.

That list is roughly the inverse of where default gym attention goes. It is also not a claim that training these areas fixes back or neck pain — that is a clinical question with a much messier literature. What two sessions a week can look like

The guideline says two days, moderate or greater intensity, all major muscle groups. It does not say a gym, a barbell, or a programme with a name. Bodyweight, resistance bands, machines and free weights all satisfy it.

The structure that covers the most ground in the least time is a small number of movement patterns rather than a long list of exercises:

  • Something that pushes — press-ups, an overhead press, a bench or machine press. Covers chest, shoulders and triceps in one movement

  • Something that pulls — rows, pull-downs, inverted rows. This is the one that addresses the upper back a seated day leaves lengthened, and it is the most relevant single item on the list

Five patterns, two sessions a week, thirty to forty minutes each. That satisfies the guideline, and the second item is the one to keep if the session gets cut short.

Getting it to actually happen

Two sessions a week is a small commitment that a large majority of desk workers nonetheless fail to sustain, and the reasons are predictable enough to design around.

The first is that strength training has an unusually high setup cost relative to its duration. A forty-minute session can easily consume ninety minutes once travel, changing and the rest are counted, which makes it fragile against a bad week in a way that a walk is not. The mitigation is to reduce the friction rather than the session: equipment at home, however minimal, removes most of the overhead, and a pull-up bar plus a pair of adjustable dumbbells covers four of the five patterns above.

The second is that progress is invisible for the first month or two, which is exactly the period when the habit is least established. Aerobic work gives immediate feedback — you feel it that day. Strength adaptations are slower and largely neural at first, so nothing looks different and nothing feels dramatically easier for several weeks. Knowing that in advance is most of the defence against concluding it is not working.

The third is scheduling collision. Both sessions and the daily walk compete for the same evening hours, and in practice the strength work loses because it is the one that requires deciding. Fixing the days rather than the times helps more than people expect: two named days a week, whatever hour they end up happening, removes the daily negotiation that the habit keeps losing. A last note on intensity, since the guideline specifies moderate or greater. That means the last few repetitions of a set should be genuinely difficult. A session that never approaches that is pleasant and does not clearly meet the recommendation — which is worth knowing, because the commonest failure among people who do turn up is not frequency but effort, and it is invisible from a training log.

What this is not

Three claims worth refusing, because they circulate widely in developer-health writing and none is well supported.

That specific exercises correct posture. Posture is highly variable between individuals, the relationship between posture and pain is much weaker than commonly assumed, and interventions aimed at changing it have not produced consistent results. Train because general strength is recommended, not because you are straightening something.

That strength training prevents RSI. The reviews do not establish that computer work causes carpal tunnel syndrome in the first place, and intervention evidence for upper-limb complaints is limited and inconsistent. That it substitutes for the walking. It does not. Ekelund's offsetting finding is about moderate-intensity aerobic activity, and the WHO lists the two recommendations separately for a reason. Someone doing two strength sessions a week and nothing else has done half of what the guideline asks.

There is a fourth claim worth handling separately, because it is the one that sounds most reasonable: that desk workers should train differently from everyone else. The WHO guideline does not say that, and neither does the evidence underneath it — the recommendation is the same for adults regardless of occupation. What varies is which parts of it you are already incidentally getting. A labourer arrives with a great deal of loading already done and a deficit in structured aerobic work; a software engineer arrives with neither, and the practical consequence is simply that both halves of the guideline have to be done deliberately rather than that the content of them changes. It is that a recognised international guideline lists two things, most people in this profession do neither, and the second one takes about eighty minutes a week.
