The pitch for a standing desk is that sitting all day is harming you and standing is the remedy. The first half is roughly defensible. The second half does not follow, and the reason is a definition.
Standing is not activity
The strongest evidence in this area is Ekelund and colleagues' harmonised meta-analysis of 1,005,791 people across 16 studies, published in the Lancet. Their finding was that roughly 60 to 75 minutes a day of moderate-intensity activity appeared to eliminate the increased mortality risk associated with high sitting time.
Moderate intensity has a definition: raised heart rate and breathing, while still able to hold a conversation. A brisk walk qualifies. Standing at a desk does not — it burns marginally more energy than sitting and produces essentially none of the cardiovascular response the study was measuring. Substituting standing for sitting changes your posture. The offsetting result is about your activity. Those are different variables, and the desk moves the one the study wasn't about.
This is the central confusion in how these desks are marketed, and it isn't a fringe misreading — it's the standard one. The research on sedentary behaviour gets cited to sell a device that does not address the mechanism the research identifies. There is a second reason the substitution is tempting, beyond the marketing. A desk is a one-off decision that requires nothing of you afterwards, and an hour of daily walking is a decision you have to make again every day for the rest of your life. Given the choice between a purchase and a habit, almost everyone prefers the purchase, and the health-equipment market exists largely to serve that preference. Recognising the trade does not make the habit easier, but it does stop the purchase from feeling like it settled something.
What the desk does do
Something real, but smaller and in a different category: postural variation.
Holding one position for hours loads the same tissues continuously. Changing position redistributes that load. This is intuitively sensible, it matches what people report, and the evidence for it is weak — the musculoskeletal literature on computer work has small samples, short follow-ups, and a persistent problem separating discomfort from pathology.
So the honest claim is that alternating between sitting and standing probably helps with discomfort, that discomfort is worth addressing on its own terms, and that this is not the same claim as the mortality one.

What to buy instead, if you only buy one thing
Since the money is going somewhere, it is worth ranking the options by how well the evidence supports each, rather than by how prominently they are marketed at developers.

The ordering is deliberately uncomfortable, because the two items with the best support cost nothing and the items with the worst support are the ones with a marketing budget. That is not a conspiracy — it is just that walking has no vendor.
It also reflects a real asymmetry in how these things get studied. A walk is easy to randomise and measure; a chair is not, because you cannot blind anyone to which chair they are sitting in and comfort is largely self-reported. So some of the weakness in the equipment evidence is a property of the research being hard rather than of the equipment being useless, and that is worth holding in mind before dismissing a purchase that makes your day more comfortable.
Standing all day is its own mistake
Worth stating because the enthusiastic version of this often ends with someone standing for eight hours and concluding that their back hurts because they aren't doing it right.
Prolonged standing is a recognised occupational exposure with its own associated problems — lower-limb discomfort, venous issues, foot and back pain. Trades that involve standing all day have known musculoskeletal profiles, and they are not empty. Replacing eight hours of one static posture with eight hours of a different static posture is not obviously an improvement. Which means the useful device is one that alternates, and the useful behaviour is alternating. A desk that goes up and down but never does is furniture.
Why the research on this is so thin
Given how many of these desks have been sold, it is reasonable to ask why there is not a clean answer, and the reasons are structural rather than a gap somebody could fill next year.
Blinding is impossible. You cannot run a trial in which participants do not know whether they are standing, so every reported outcome that depends on how someone feels is contaminated by their expectation about the device — and expectation effects are large for exactly this kind of self-reported comfort measure. The people who buy standing desks generally believe they will help, which is the worst possible starting condition for measuring whether they do.
The outcomes that matter also arrive too late to study directly. The mortality and cardiometabolic questions play out over decades, so nobody is running a thirty-year randomised trial of desk height. What gets studied instead are short-term surrogates — postural measurements, discomfort scores over weeks, energy expenditure — and the leap from those to a long-term health claim is exactly the leap the marketing makes and the research does not.
Add the healthy-adopter problem: people who buy sit-stand desks tend to be people already inclined to do other things for their health, so any observational comparison between owners and non-owners is measuring the person rather than the furniture. Put those three together and it becomes clear why the honest verdict is 'probably pleasant, unproven, and not the intervention with the evidence' rather than a number.
Making it actually happen
The failure mode is well documented in the least surprising way possible: people buy the desk, use it enthusiastically for a fortnight, settle on one height, and never move it again.
Attach the height change to an event that already happens — a build running, a meeting starting, a deploy going out. A change that depends on remembering will not survive a busy week
Target the long unbroken blocks specifically. Meyer et al. recorded a developer coding for 135.7 minutes with no break over two minutes; that pattern is what this device is for, not the fragmented afternoon
Get the monitor height right in both positions. A standing setup with the screen too low produces neck flexion for hours, which is a new problem rather than a solved one
Use a mat if you stand for extended periods, and wear something other than the shoes you have been sitting in. Hard floors are a large part of why standing gets uncomfortable
Do not let the purchase substitute for the walk. This is the important one, and it is the failure the evidence actually predicts
The last point is the whole argument compressed. There is a version of buying a standing desk that is a genuine improvement to a working day, and a version that is an expensive way to feel like the sedentary problem has been addressed while the variable with a million-person study behind it stays exactly where it was. None of this is medical advice, and the WHO's own position is worth repeating as a check on anyone's confidence here: reducing sedentary behaviour is recommended, and the evidence was insufficient to say how much.
