Somewhere in your working life, someone corrected your posture. A parent, a physio, a coworker glancing over at your screen. The instruction is always the same: sit up straight, shoulders back, spine neutral. It's delivered with total confidence, as though it were established medical fact rather than an inherited habit.
The physiotherapy researchers who actually study this for a living have gone looking for the evidence behind that confidence. Repeatedly. And the honest summary of what they found is uncomfortable for an entire industry: there is no strong evidence that one static sitting posture prevents back pain, and no strong evidence that a different one causes it. What the researchers who study this actually found
Peter O'Sullivan is one of the most cited researchers in musculoskeletal pain, and his group has spent years pushing back on the 'ideal posture' model from inside the profession that invented it. In 2019, Diane Slater, Vasileios Korakakis, Peter O'Sullivan, David Nolan and Kieran O'Sullivan published a viewpoint in the Journal of Orthopaedic & Sports Physical Therapy titled, pointedly, "Sit Up Straight": Time to Re-evaluate. Their argument wasn't that posture is meaningless. It was that the belief connecting a specific spinal alignment to the cause or prevention of pain has outrun what a formal search of the evidence supports.
That 2019 piece didn't appear from nowhere. O'Sullivan's team had already conducted a formal search specifically for evidence that posture makes a measurable difference to back pain outcomes, and came back without it. Later studies in the same tradition found no consistent association between habitual sitting posture and back pain in either direction. That's a stronger claim than "the evidence is mixed." It's closer to: the field went looking for the thing everyone assumed was proven, and it wasn't there in the form people assumed.
A separate and larger piece of work backs this up at a bigger scale. In 2020, Christopher Swain, Fumin Pan, Patrick Owen, Hendrik Schmidt and Daniel Belavy published a systematic review of systematic reviews in the Journal of Biomechanics. They screened 4,285 publications down to 41 systematic reviews covering spine posture, physical exposure at work, and low back pain, and asked a blunt question: across all this existing review-level evidence, is there consensus that any specific posture or physical exposure causes low back pain? The answer was no. Not "weak evidence for." No consensus, full stop, across four decades of accumulated review literature.
Forty-one systematic reviews, and no agreement that any single spine posture is the culprit.
It's worth being precise about what "no consensus" means here, because it gets misquoted in both directions. It doesn't mean researchers checked and found posture is irrelevant to pain. It means that across dozens of reviews using different definitions of posture, different populations, and different study designs, no single causal story about a specific spine angle has held up consistently enough for the field to agree on it. That's a statement about the state of the evidence, not a tidy negative finding you can print on a poster. Ambiguity of that kind is less satisfying than either "posture matters enormously" or "posture is a total myth," which is probably part of why the plainer, more confident claim keeps winning in gyms, ergonomics brochures and parenting advice. Then why does everyone still say the same thing?
This is where the story gets more interesting than "the science was wrong." A separate line of survey research asked practicing physiotherapists what they consider optimal sitting posture. More than 85% picked an upright position. Roughly 1% picked something more flexed or relaxed. The research literature on this gap is fairly direct about the explanation. Clinicians weigh dominant clinical opinion and their own training more heavily than they weigh the primary evidence, which is a normal and human failure mode, not a scandal specific to physiotherapy. It's the same reason so much workplace advice persists years after the study it was based on has been reinterpreted or retracted in spirit if not in print.

The idea that's actually held up: the next posture, not the right one
If a single correct posture isn't the answer, what replaces it? The phrase that's become something of a slogan in ergonomics circles is "the best posture is the next posture." It's catchy, but the mechanism behind it is fairly mundane and much better supported than the posture-causes-pain claim it's replacing. If the mechanism really were about spine angle, the corrected posture should feel progressively better the longer it's held, since it's the one supposedly protecting you. That's consistent with a static-loading explanation and inconsistent with a single-correct-angle explanation, and it's the kind of everyday detail the O'Sullivan-style critique of posture dogma keeps pointing back to.
Where the caution has to go
None of this should curdle into the opposite overclaim — that posture is entirely irrelevant and nothing about how you sit matters. What actually changes if you take this seriously
The practical shift is smaller than the research sounds. You don't need a new chair, a new desk angle, or a corrective brace. You need to stop treating any single sitting position as the finish line and start treating variation itself as the goal.
Stop chasing one ideal spine angle and start noticing how long you've held any position, upright included
Change position every 20-30 minutes rather than trying to perfect a single one you sustain for hours
Treat a standing option, a recline option, and an upright option as three tools to rotate through, not as a hierarchy with one correct choice
Save actual posture correction effort for positions that are genuinely extreme (very prolonged spinal flexion under real load), not for ordinary slouching during a video call
If pain is already present and specific, that's a question for a clinician assessing your actual body, not a question this kind of population research can answer for you
This is general information drawn from published research, not a diagnosis or a treatment plan, and if something already hurts, a clinician who can examine you is worth more than any systematic review. But the systematic reviews are clear on one thing at least: the industry-standard instruction to sit up straight was never built on the evidence people assumed backed it, and the more defensible habit is simply not staying still.
There's a reason this particular myth has proven so durable, and it's worth naming honestly rather than treating the whole industry as bad faith. Posture is visible in a way movement volume isn't. A manager can glance at a slouched employee and form an instant judgment; nobody can eyeball how many minutes someone stood up today. Advice tends to calcify around whatever is easiest to observe and correct in the moment, regardless of whether it's the variable the evidence actually implicates. That's a bias worth watching for in yourself too — it's much easier to fix your posture for the five seconds someone's watching than to actually get up and walk for a minute every half hour, and the second one is the habit with better evidence behind it.
